Additional verification documents may be required after you submit your application. Our enrollment specialist will contact you after you submit your application if any additional document are needed.

• Determination of Family Size (Hardship)

• Family Support Letter

• Income Explanation Letter

• Eligibility Verification Authorization Form

• LIC 701 - Physician's Report

• Self Declaration of Adult Living in the Home

• Tribal Member Declaration Form

• Zero Income Statement 

Send your application with immunization record, TB clearance for preschool age child(ren) only, copies of birth certificates for each child in family, and proof of all income to:
